To make a Workflow Process Flowchart, begin by either creating one from scratch using PPT, or downloading the Excel template from the link at the bottom of the ASQ website.  To enable Macros when you download, save as “macro enabled excel template”.  http://asq.org/learn-about-quality/process-analysis-tools/overview/flowchart.html.  

2. DEFINE THE Current Workflow PROCESS that needs improvement.  The workflow should pertain to information technology related to nursing clinical care or nursing administration.  (See some examples https://healthit.ahrq.gov/health-it-tools-and-resources/evaluation-resources/workflow-assessment-health-it-toolkit/all-workflow-tools/flowchart) (Links to an external site.)

3. DETERMINE ALL INDIVIDUALS, DEPARTMENTS, AND GROUPS INVOLVED in the process. These are referred to as “players.”

4. BRAINSTORM THE STEPS in the process. The specific sequence is less important than determining all of the steps at this point (although thinking sequentially may help identify any missing steps).

5. To show your recommended new, improved workflow, CONSTRUCT THE FLOWCHART GRAPHICALLY using rows or columns corresponding to the associated work units (e.g., provider, nursing). Ideally this will be laid out in a sequence in which players working with one another will be adjacent to each other.

6. ARRANGE THE STEPS SEQUENTIALLY. Place the card with the first step in the column or row that corresponds to the player responsible for it. The second step should be placed further along in the process to show that it occurs later in time, opposite that step’s player. Continue in this fashion until all the steps have been placed in the respective lane of the responsible player.

7. DRAW ARROWS between cards to show the process flow. Keep in mind that some steps may involve two players: “Ted telephones Mary.” For these, make a second card to place opposite the second name. Write the action from the second player’s point of view: “Mary receives a phone call.”

Post your Workflow and a one page instruction for the staff that includes the following information:

1. Describe the current workflow.
2. Describe what you want to achieve with information technology. 
3. Describe the workflow after the requested changes, as depicted in your template.
4. What products or services will be needed to make the changes?
5. Who are the key internal or external “customers” of the changes?
6. What could staff do to facilitate this change process?
